Patellofemoral pain syndrome: a critical analysis of the biomechanical mechanisms associated with pain and conservative rehabilitation

Abstract

BACKGROUND: Patellofemoral Pain Syndrome (PFPS) is a multifactorial condition characterized by anterior knee pain, frequently associated with biomechanical, neuromuscular, and functional alterations of the patellofemoral joint.
OBJECTIVE: The present study aimed to critically discuss the main biomechanical factors involved in the development, perpetuation, and rehabilitation management of PFPS.
METHODS: This study consists of a structured narrative literature review conducted through searches in the PubMed/MEDLINE, Scopus, SciELO, and Google Scholar databases. After the selection process, 48 studies published between 1979 and 2024 composed the final corpus of analysis.
RESULTS: Evidence related to patellofemoral biomechanics, altered movement patterns, and conservative treatment strategies was included. The findings demonstrate that factors such as increased patellofemoral compressive forces, dynamic genu valgum, muscular strength deficits, and alterations in neuromuscular control present biomechanical plausibility for contributing to symptom manifestation. However, the relationship between these alterations and pain appears to be multifactorial and non-linear, considering that many of these patterns may also be present in asymptomatic individuals.
CONCLUSION: Regarding rehabilitation, multimodal approaches involving muscular strengthening, motor control, and individualized load progression demonstrated the strongest scientific support for the conservative management of the condition.
