The use of medicinal plants in the management of arterial hypertension and diabetes mellitus is a complementary strategy that enriches the scope of clinical care, strengthens the autonomy of subjects and expands therapeutic possibilities in the territory.This study aimed to report the experience of the process of creating educational folders for rational and safe use of medicinal plants for people with systemic hypertension and diabetes mellitus attended in primary health care. This is action research with exploratory approach in which a bibliographic review and virtual meetings were carried out to create four educational folders focusing on the preparation methods, benefits, and contraindications of medicinal plants. Individual and collective educational actions were carried out associated with the distribution of folders. The results highlighted that the creation of educational materials is a strategy that allows the democratization of access to knowledge about phytotherapy, promoting the autonomy of individuals, based on evidence aligned with public health practices. The educational materials converge to clarify the particularities of medicinal plants for systemic hypertension and diabetes mellitus, serving as an important source of knowledge and practices for strengthening the link between health education and primary health care.
