Abstract

The Unified Health System (SUS) aims to guarantee comprehensiveness and equity in Primary Health Care (PHC), making the identification of family vulnerabilities and biopsychosocial determinants essential. This is achieved through tools such as the Coelho-Savassis Family Risk Scale (ERF-CS), the Genogram, and the Ecomap. A case study applying these instruments to the S.M. and M.B. family, users of a Basic Health Unit (UBS) in Foz do Iguaçu, revealed that S.M. suffered from chronic illnesses (skin cancer, diabetes, hypertension) and, although she had a support network, faced significant barriers to accessing specialized services. The assessment determined that the family was at a high-risk level (10 points) according to the ERF-CS, while the Genogram and Ecomap confirmed the comorbidities and the existence of strong support networks alongside difficulties accessing care. In conclusion, the joint application of the ERF-CS, the genogram, and the ecomap proved crucial for a holistic assessment of social vulnerability and biopsychosocial factors, allowing the visualization of patterns, networks, and barriers, which is essential for designing a comprehensive and equitable care plan in primary health care and thus fulfilling the equity commitment of the SUS.
