We aimed to analyze the mortality rate from prostate cancer in agricultural workers residing in Paraná between 2003 and 2020. An ecological study was conducted consisting of the deaths by prostate cancer recorded in the Mortality Information System. Adjusted mortality rates (per 100,000) were calculated through the direct method, using as reference the number of agricultural workers from the Annual Report of Social Information in 2010. The mortality rate went from 246.18 deaths per 100 thousand agricultural workers in 2003 to 211.97/100 thousand in 2020. There was a significant (p=0.043) decreasing trend of -1.89 cases/100 thousand each year. The mesoregions of Paraná with the highest rates were the Southeast and Southwest. We conclude that there was a reduction in mortality among agricultural workers, but the population is vulnerable to the disease, since the rates were high when compared to other occupations. It is expected to help in the understanding of the behavior of this indicator, providing information for the development of public health and development policies that consider the specificities of the disease in its social, political and economic context, with emphasis on the reduction and control of occupational risk factors, contributing to the achievement of goal 3 'health and well-being' of the Sustainable Development Goals.
